What is depression

Depression (clinical depression, or major depressive disorder) is a common mental health condition that causes persistent low mood and a loss of interest or pleasure in daily life for two weeks or longer. It ranges from mild to severe and often occurs alongside anxiety. With the right treatment, most people recover or see a meaningful reduction in symptoms.

Depression affects mood, thinking and the body. You may recognise several of the following, most of the day, nearly every day:

  • Persistent sadness, low mood or a sense of emptiness
  • Loss of interest or enjoyment in activities you used to like
  • Fatigue, low energy or feeling slowed down
  • Changes in sleep — difficulty sleeping or sleeping too much
  • Changes in appetite or weight
  • Difficulty concentrating, remembering or making decisions
  • Feelings of worthlessness, guilt or hopelessness
  • Irritability, tearfulness or restlessness
  • Loss of libido
  • Thoughts of self-harm, death or suicide

Depression usually results from a combination of factors rather than a single cause, including genetic and family history, brain chemistry, long-term stress, bereavement or loss, physical illness, hormonal changes, and alcohol or drug use.

Psychiatric assessment and diagnosis

Effective treatment starts with an accurate diagnosis. A full psychiatric assessment clarifies whether your symptoms are due to depression, a related condition such as bipolar disorder or anxiety, or a physical cause — so treatment is targeted rather than trial and error.

Medication review and optimisation

Antidepressants can be highly effective for moderate to severe depression. If you are already taking medication that is not working, Dr Kurkar can review your diagnosis, dose and choice of medication, and adjust or switch treatment safely where appropriate.

Psychological therapy

Talking therapies such as cognitive behavioural therapy (CBT) are recommended for many people with depression, either on their own or alongside medication. Where therapy is indicated, we can advise on and signpost suitable options.

TMS therapy for treatment-resistant depression

If depression has not responded to two or more antidepressants, you may have treatment-resistant depression. Transcranial Magnetic Stimulation (TMS) is a non-invasive, medication-free treatment recommended by NICE for treatment-resistant depression. It uses magnetic pulses to stimulate areas of the brain involved in mood regulation, and does not require anaesthetic or sedation. Dr Kurkar was a principal investigator on the BRIGhTMIND TMS trial.

Lifestyle and ongoing support

Treatment-resistant depression

Around a third of people with depression do not fully respond to the first treatments they try. This is known as treatment-resistant depression, and it does not mean your depression cannot be treated — it means a more specialist approach is needed.
